This session focused on increasing healthcare agencies' involvement in strategies that address social determinants of health (SDOH). More recently, health systems have recognized the importance of addressing underlying determinants of health in order to improve well-being for all individuals in a community.
Participants learned about this trend and gained insights from work with communities in North Carolina that are funded by the Blue Cross Blue Shield of North Carolina Foundation, as well as from partnerships with healthcare institutions formed to address SDOH. Attendees also heard examples from Urban Health Plan’s initiatives in the Bronx that stretch beyond traditional clinic-based services and aim to improve community conditions in pursuit of better health.
