We help communities and those who invest in them build capacity to make policy, systems, and environmental changes.

Healthy Places by Design is a trusted partner to philanthropy, nonprofits, associations, community leaders, and coalitions, with deep expertise in diverse community contexts. Partner with us to enhance collaboration, cultivate social connection, and accelerate local action.

Our Services and Programs

Relationships are the heart of our work, and our bottom line is helping community coalitions, including formal and informal leaders, create lasting change at local, state, regional, and national levels.

We bring decades of experience supporting diverse community partnerships and leaders in more than 40 states as they implemented equity-focused policies, created welcoming public spaces, and transformed their communities to improve health and wellbeing for all. We will work with you to co-develop an effective plan of action with approaches ranging from standalone services and programs to an integrated strategy that addresses complex project goals, community contexts, and partner needs. Community Leadership and Action

We help community leaders and partnerships effectively work together using an inclusive, equity-focused lens. We provide individualized support to deepen collaborative leadership, address emerging needs, and apply tools in real time. Our evidence-based, practice-tested Community Action Model provides a deep well of resources and concepts from which our services are tailored, including Essential Practices and an Action Cycle for meaningful and lasting changes to policies, systems, and environments.

We advance peer and group learning and networking experiences by tailoring our support to specific initiatives, organizational goals, and participant needs. Our team co-creates welcoming spaces, structures, and activities that foster innovative thinking, develop new partnerships, and inspire sharing of wisdom and resources.

Field Building

We strengthen the healthy communities field by increasing capacity for collective progress and systems change. Our team creates and curates original content to inform and inspire the field, including assessment and recommendation reports, practical guides, thought leadership commentary and blogs, and on-demand virtual and in-person learning and networking.

Community Collaboration

We believe that when people work together, their collective ideas and efforts create greater change than any individual could achieve alone.

Healthy Places by Design deepens the capacity of individuals, groups, and organizations to work together on improving their community’s policies, systems, and environments. Our collaboration services include tailored learning and networking experiences, cohort support, and our unique Collaboration Lab course, all of which build the capacity of current and future leaders to work across sectors and with residents to create thriving communities.

Socially Connected Communities

We are growing a movement to integrate social connection as a vital component of healthy and equitable communities.

Trusting and meaningful relationships enhance our mental, physical, and emotional health and well-being. In fact, stronger social connections can boost a person’s likelihood of survival by 50 percent. Through the Socially Connected Communities Network and other services, we help philanthropic and nonprofit organizations weave strategies for social connection into their work.

Community Action Model

We translated decades of successful initiatives into a comprehensive model to describe the community change process and to guide community action.

In our collaborative work with community leaders and residents, their funders, and other key partners, our Community Action Model guides our way. The model highlights the importance of a community’s context, defines six essential practices for success, and outlines a 3P Action Cycle: Partner, Prepare, and Progress. It represents more than 20 years of learning and refinement by Healthy Places by Design and is a well-tested approach for local investments.

Since 2002, our organization has supported more than 200 community partnerships in more than 40 states, working on behalf of dozens of philanthropic, nonprofit, and government organizations.
